We have prepared for you on-line application form at Club show with the award title Club Winner of 12 April 2014 in the exhibition premises Krásná louka, Mladá Boleslav. All fields must be completed (except for competitions and classes). This application form is for one dog only. The applicant shall make a photocopy for any other dog application. Fees are payable at local currency or in € in cash only at the show registration counter.
Before submitting your application, please refer to provisions. A copy of the pedigree should be attached to the Application form. For the winner class please attach also a copy of the Interchampion, Champion, National winner, the Club winner and the Special Show winner document (diploma, attest etc.) (field "Annex").
The undersigned states that he/she agrees to the provisions of the Club Show.
